Transition Care Program (Bed-Based)
Helping you transition from hospital to home
The Transition Care Program (TCP) provides short-term support for older people who have completed their hospital treatment but need additional time and assistance before returning home or making longer-term care arrangements.
Delivered in a supportive healthcare setting, the program focuses on helping you regain your independence, improve your confidence, and plan for the next stage of your care.
You may be eligible for the Transition Care Program if you:
- Have completed your hospital treatment
- Have been assessed as eligible through an Aged Care Assessment
- Would benefit from additional support before returning home or moving to another care setting
Your healthcare team will discuss the program with you if it is appropriate for your circumstances.
Every person’s recovery is different. Your care plan will be developed with you and based on your individual goals.
Depending on your needs, your care may include:
- Nursing care and clinical support
- Physiotherapy and exercise physiology to improve strength, mobility, and confidence
- Occupational therapy to help you regain independence with everyday activities
- Dietetic support to assist with your nutritional needs
- Speech pathology, if required
- Medication review and education
- Support from a social worker
- Planning and coordination of services to support your return home
