Sometimes you may no longer need acute hospital treatment but still need additional care and support before returning home.

Our Subacute Care service provides a supportive environment where you can continue your recovery, rebuild your strength and confidence, and work towards returning to your everyday activities.

Our focus is on helping you achieve your recovery goals while maintaining your independence and quality of life.

Our Subacute Care service provides a supportive environment where you can continue your recovery, rebuild your strength and confidence, and work towards returning to your everyday activities.

Our focus is on helping you achieve your recovery goals while maintaining your independence and quality of life.

Subacute Care may be suitable if you are:

  • Recovering from an illness, injury, or surgery
  • Regaining your strength before returning home
  • Recovering after a stay in hospital
  • Working towards becoming more independent with everyday activities

Your healthcare team will discuss with you whether Subacute Care is the right option for your needs.

Your care will be tailored to your individual goals and delivered by a multidisciplinary team of healthcare professionals.

Depending on your needs, your care may include:

  • Medical and nursing care
  • Physiotherapy to improve strength, balance, and mobility
  • Exercise physiology to build strength, endurance, and confidence through individualised exercise programs
  • Occupational therapy to support everyday activities and prepare for a safe return home
  • Dietetic support to assist your nutrition and recovery
  • Speech pathology, if required, to support swallowing or communication
  • Medication reviews and advice from a pharmacist
  • Support from a social worker, if needed, to assist with planning and connecting you with community services

Throughout your stay, we’ll work closely with you and your family or carers to ensure your care reflects your goals and supports your recovery.

Planning for your return home begins early during your stay. Before you leave, we’ll work with you to ensure you have the information, equipment, and support you need to continue your recovery safely.

Where required, we’ll help connect you with community health, home support or other local services to support your ongoing care.
