Social Work
Our Social Worker can provide support with emotional, financial, or social difficulties, including living with life changes brought on by illness and can connect patients with services and resources to help them cope.
These services include:
- Providing patient education and counselling
- Advising and educating family care givers, to assist them in understanding, dealing with, and supporting the client or patient.
- Making referrals to other services and coordination of these
- Address legal issues, such as assisting with VCAT hearings relating to their patients.
- Assist with the planned discharge from care facility to home or other care facility.
- Advocate for clients or patients to resolve crises.
- Organising support groups.
- Identify environmental impediments to client or patient progress.
- Linkages to mental health & alcohol and other drug services
- Housing support & referral, including alternative accommodation and respite options.
- Future planning including information and assistance with advanced care planning.
- Information about Centrelink, TAC, WorkCover payments and services.
- Referral to and information on financial and legal services and options.
We do accept referrals from Doctors and other Health Professionals, however, referrals are not necessary. Please contact our Reception on (03) 5558 6000 to enquire.
