Palliative Care

At Timboon and District Healthcare Service (TDHS), we provide compassionate, person-centred palliative care for consumers, families, and carers living with a life-limiting illness.

Our focus is on supporting comfort, dignity, and quality of life, while ensuring consumers and their loved ones feel informed, supported, and involved in care decisions.

We understand that every person’s journey is unique. Our team works closely with consumers, families, and carers to deliver care that respects individual wishes, values, cultural needs, and personal preferences.


What is Palliative Care?

Palliative care supports people living with a serious or terminal illness by focusing on:

  • Symptom management
  • Pain relief and comfort
  • Emotional and psychological well-being
  • Social and spiritual support
  • Care planning and decision-making
  • Support for families and carers

Palliative care can be provided alongside active treatment and is not limited to end-of-life care.

Where appropriate, TDHS aims to support consumers to receive care in a comfortable and familiar environment, helping people remain connected to their home, family, and community.

TDHS also offers a dedicated palliative care room designed to provide a calm, private, and supportive environment for consumers and their loved ones.

The room has been thoughtfully designed to promote comfort, dignity, and family connection during all stages of care. Families are encouraged to spend time with their loved one in a welcoming and peaceful setting, with space that supports privacy, rest, and meaningful time together.

Our dedicated palliative care room reflects TDHS’s commitment to providing compassionate end-of-life care close to home, allowing consumers to remain connected to their local community and support networks during this important time.


End-of-Life Care

During end-of-life care, our focus remains on comfort, dignity, and ensuring the individual’s wishes remain central to care delivery.

Our staff are committed to providing compassionate care and support during this important time, ensuring consumers and families feel respected, informed, and supported.

Referrals and Access

Consumers may access palliative care services through:

  • Presentation to the TDHS Urgent Care Centre
  • Direct admission through the TDHS medical clinic
  • Referral from a general practitioner or specialist
  • Referral from hospitals or healthcare services
  • Referral from community health providers

Following referral or admission, our team will work alongside the consumer, their family, and treating healthcare professionals to assess individual needs and develop an appropriate care plan.
