District Nursing Service
TDHS District Nursing provides professional nursing care and support in the community to help you maintain your health, independence, and wellbeing at home.
We work in partnership with you and, where appropriate, your family, carers, and other healthcare providers to deliver care that is safe, respectful, and tailored to your individual needs, preferences, and goals.
Our services may be available through a range of funding programs or private arrangements, depending on eligibility and clinical need.
Services we provide
Our district nursing team provides a range of clinical nursing services, including:
- Wound assessment and management
- Medication management and clinical monitoring
- Catheter care
- Diabetes support and education
- Injections
- Skin care and pressure injury prevention
- Continence support
- Falls risk assessment and prevention support
- Palliative and end-of-life nursing care
- Post-acute nursing care and short-term clinical support following hospital discharge
- Brokered nursing services and short-term clinical care in partnership with other health providers
- Pathology collection
- Personal care support related to clinical nursing needs
- Health assessment, education, and self-management support
Services are provided based on assessment, eligibility, and clinical need.

We work closely with other healthcare providers and community services to support coordinated care, including:
- General practitioners (GPs)
- Hospitals and specialist services
- Allied health professionals
- Community support services
- Local care providers
- Aged care and disability support providers
Where needed, we can assist with referrals to services such as:
- Aged care assessment services
- Physiotherapy
- Occupational therapy
- Podiatry
- Dietetics
- Diabetes education
- Speech pathology
- Audiology
- Social and community support services

Referrals and access
Referrals can be made by:
- You, your family, or carers
- General practitioners (GPs)
- Hospitals
- Other health professionals
- Aged care, disability, or community service providers
Self-referrals are welcome.
Following referral, a district nurse will complete an assessment to understand your health needs, goals, and support requirements. If the service is appropriate, we will work with you to develop an individualised care plan.
Fees may apply depending on the program through which your care is funded.
