Acute Medical Care

Caring for you when you need hospital care

If you need to stay in hospital because of an illness, injury, or after surgery, our team is here to support you.

At Timboon and District Healthcare Service (TDHS), we provide high-quality hospital care in a welcoming, community-based environment, allowing you to recover close to home and the people who matter most.

You may be admitted to our Acute Care ward:

  • Following assessment in our Urgent Care Centre
  • Through a referral from your GP
  • Following transfer from another hospital
  • As part of a planned admission

Our team will explain your care, answer your questions, and keep you informed throughout your stay.

While you’re in hospital, our team will work with you to support your recovery and help you return home safely.

During your stay, you can expect:

  • Care from our experienced doctors, nurses, and healthcare team
  • Regular reviews of your health and recovery
  • A personalised care plan based on your individual needs
  • Support to maintain or regain your strength and independence
  • Access to allied health professionals, such as physiotherapists, occupational therapists, and dietitians, if needed
  • Involvement in decisions about your care, with family or carers included where appropriate

We’re committed to providing safe, compassionate care and ensuring you feel informed, supported, and respected throughout your stay.

Planning for your return home begins early during your admission. Before you leave the hospital, we’ll work with you and your support network to ensure you have everything you need for a safe transition home.

This may include:

  • Information about your recovery
  • Advice about medications
  • Follow-up appointments
  • Referrals to community or support services, where required

Our goal is to help you return home feeling confident and well supported.

To help make your stay as comfortable as possible, please bring:

  • Your Medicare card and any private health insurance details
  • A list of your current medications or your medications in their original packaging
  • Comfortable clothing, sleepwear, and supportive footwear
  • Personal toiletries
  • Glasses, hearing aids, dentures, or mobility aids, if you use them
  • Any advance care planning documents, if you have them

We recommend leaving valuables and large amounts of cash at home whenever possible.
